A pulmonary ventricle system producing pulsatile pressure in single ventricle: experimental model

چکیده
Objective: Patients with single functional ventricles have a bad prognostic; most result in heart failure even after operation. The right heart bypass operations, except for right atrium and right ventricle connections, lack pulsatile flow and the volume returning to the common ventricle is low. To solve this problem, we tried to experimentally create a pulmonary ventricle which produces pulsatile pressure. Methods: We tried to form a right ventricle system which produces pulsatile pressure. The experimental model was used in six sheep. The pressures of the created pulmonary ventricle, pulmonary artery and the left ventricle were measured after removing the cardiopulmonary bypass. Results: The mean systolic and diastolic pressures of the pulmonary artery were 15,6 ± 2.0 mmHg and 4.5 ± 1.5 mmHg, respectively. The mean left ventricular systolic pressure was 76.6 ± 4.4 mmHg. Conclusion: A ventricle producing pulsatile pressure is necessary for regulating the pulmonary artery flow with high central venous pressure and low non-pulsatile pulmonary pressure in anomalies with single functional ventricles. متن کاملA Newborn Infant with a Pulsatile Substernal Structure in a Midline Defect; Cantrell's Syndrome
The present report describes a male newborn with a pulsatile structure beneath the sternum. Echocardiography showed common atrium, a single ventricle, mitral atresia, double outlet right ventricle, subpulmonary stenosis, small pulmonary artery branches, and a thin walled ventricular diverticulum suggestive of Cantrell's syndrome. The diverticulum was resected to prevent life threatening events ...
متن کاملCoupling pediatric ventricle assist devices to the Fontan circulation: simulations with a lumped-parameter model.
In pediatric ventricular assist device (VAD) design, the process of matching device characteristics and dimensions to the relevant disease conditions poses a formidable challenge because the disease spectrum is more highly varied than for adult applications. One example arises with single-ventricle congenital defects, which demand palliative surgeries that create elevated systemic venous pressu...
متن کاملDynamics of a cardiovascular model obtaining measurable pulsatile pressure output∗
In this study, a lumped compartment cardiovascular model is developed to predict pulsatile pressures in the systemic and pulmonary circulation as well as in finger arteries. Essential compartments of the model include arterial and venous pulmonary, left and right ventricles, and arterial and venous systemic compartments. A pulsatile left ventricle is incorporated to be the source of pulse waves...

Single ventricle heart defects involve pathologies in which the heart has only one functional pumping chamber. In these conditions, treatment consists of three staged procedures. At stage 1 pulmonary flow is provided through an artificial shunt from the systemic circulation. Representative hemodynamics models able to explore different virtual surgical options can be built based on pre-operative...
